## 🔑 核心概念

- **Infection-related GN（IRGN）**：感染誘發的免疫複合物（IC）沉積於腎絲球 → 補體活化 → GN
- 屬於 **Immune complex GN（Granular staining on IF）**：~40–45% 所有 GN
- 最典型病因：**Streptococcus（PSGN）**，但實際上多種細菌/病毒皆可引起

## 📊 主要類型

### 1. Post-Streptococcal GN（PSGN）— 最典型
| 特徵 | 說明 |
|------|------|
| 致病菌 | Group A β-hemolytic Strep（咽炎 or 膿皮病 impetigo） |
| 潛伏期 | **咽炎後 1–3 週**；皮膚感染後 3–6 週 |
| 年齡 | 兒童 > 成人 |
| 補體 | **↓ C3**（特徵性，C4 通常正常）→ 活化 alternative pathway |
| 血清學 | ASLO ↑（咽炎後），anti-DNase B ↑（皮膚感染後） |

### 2. Staphylococcus-related GN（IRGN）
- 多見於**成人糖尿病患者**或免疫抑制者
- 常見病因：bacteremia、skin/soft tissue infections、尿路感染
- 補體：↓ C3，有時 C4 也降
- 注意：**IgA dominant 的 Staph IRGN 存在**（IF 與 IgAN 類似，需鑑別）

### 3. Endocarditis-associated GN
- 亞急性細菌性心內膜炎（SBE）→ 持續菌血症 → 免疫複合物沉積
- 特徵：fever、⊕ blood cultures、valvular disease、↓ C3
- IF：granular Ig deposits（Staph 等）
- 治療：針對 endocarditis（抗生素），GN 通常隨感染控制而好轉

### 4. 其他 Infection-related GN
| 病原體 | 備註 |
|--------|------|
| **Hepatitis C（HCV）** | 常透過 cryoglobulinemia → MPGN pattern |
| **Hepatitis B（HBV）** | Membranous nephropathy（小孩），MPGN（成人） |
| **HIV** | Collapsing FSGS（最典型）；HIVAN |
| **"Shunt" nephritis** | 分流管感染（Staph epidermidis）→ IC GN |

## 🩺 臨床表現

### 典型 PSGN
- **Nephritic syndrome**：
  - 血尿（gross hematuria 或 cola-colored urine）
  - Oliguria + 水腫（浮腫）
  - HTN
  - ↑ BUN / Cr（AKI）
- 潛伏期後出現（≠ IgAN 的 synpharyngitic 同步發作）
- 多為自限性（self-limited），尤其兒童

### 成人 IRGN
- 表現可不典型，慢性 indolent 病程常見
- 糖尿病患者：容易被誤診為 diabetic nephropathy
- 心內膜炎：需同時評估全身感染症狀

## 🔬 診斷

### 血液檢查
- CBC（貧血、leukocytosis）
- Scr、BUN、eGFR
- **Complement**：**C3 ↓**（典型 PSGN 6–8 週後 C3 應恢復正常）；C4 通常正常（alternative pathway）
- **ASLO titer**（anti-streptolysin O）：咽炎後 ↑（Se ~80%）
- **Anti-DNase B**：皮膚感染後更有用（Se >90%）
- Blood culture（排除 endocarditis, bacteremia）
- HBV/HCV serology、HIV（排除其他感染）
- ANA、anti-dsDNA（排除 SLE）
- Cryoglobulin（排除 cryoglobulinemia）

### 尿液
- UA + 尿沉渣：dysmorphic RBCs、RBC casts、WBC casts
- Spot UPCR

### 腎切片（indication）
- 補體 C3 8週後仍不恢復（正常 PSGN 6–8 週恢復）
- 腎功能持續惡化（不符合自限性病程）
- 成人患者（預後較差，不確定診斷時）
- IF：**granular staining**（IgG, IgM, C3）；EM：**subepithelial "humps"**（PSGN 特徵性）

## 💊 治療

### PSGN
- **支持性治療為主**：
  - Antihypertensives（loop diuretics 控制血壓/水腫）
  - 限鹽飲食
  - Dialysis：若嚴重 AKI 需透析橋接
- **抗生素**（清除感染源）：不能逆轉已發生的 GN，但可防止傳播
  - Pen G 或 amoxicillin × 10 天（仍有活躍 strep 感染時）
- **無需 immunosuppression**（自限性疾病）
- **預後**：兒童 >95% 完全恢復；成人預後較差，30% 可有持續 proteinuria 或腎功能下降

### Endocarditis-associated GN
- **治療 endocarditis**（prolonged ABx）→ GN 多隨之好轉
- 考慮腎切片確認診斷
- Steroids 通常無效

### HCV-related GN（見 MPGN / Cryoglobulinemia 筆記）
- DAA（Direct-acting antivirals）治療 HCV → GN 多改善

## 📈 預後

| 族群 | 預後 |
|------|------|
| 兒童 PSGN | 優良，>95% 完全恢復 |
| 成人 PSGN | 較差，30–40% 有持續 HTN / 輕度蛋白尿 |
| 老年 / DM IRGN | 慢性腎功能不全風險高 |
| C3 持續不恢復 | 懷疑 C3 glomerulopathy 或 MPGN |

## ⚠️ Clinical Pearls

1. **C3 ↓ + 潛伏期 1–3 週 post-strep → PSGN**：這是 immune complex（alternative complement pathway）激活的特徵，C4 通常正常（≠ SLE/Cryoglobulinemia 的 C3+C4 都↓）。

2. **C3 8 週仍不恢復 = 非典型**：PSGN 的 C3 應在 6–8 週內恢復正常。若持續低 C3 → 考慮 MPGN、C3 glomerulopathy、SLE 等，需腎切片。

3. **成人 IRGN vs. IgAN**：Staphylococcus IRGN 可出現 IgA-dominant IF pattern，易誤診為 IgAN。鑑別點：IRGN 有活躍感染源、補體↓、EM 有 subepithelial/subendothelial deposits。

4. **Synpharyngitic hematuria = IgAN**：症狀與感染同步或 <2 天出現 → IgAN（mesangial IgA）。潛伏期 1–3 週 → PSGN（immune complex）。
